1. What is protected health information (PHI)?

Protected Health Information (PHI) is any information that identifies you and relates to your past, present, or future physical health, the services provided to you, or payment for those services. This includes your name, address, date of birth, medical history, treatment records, and any other information collected in connection with your care at Fine China Body Sculpting.

• Payment — to process billing, collect payment, or verify insurance coverage where applicable.
• Healthcare operations — for internal business activities such as quality assurance, staff training, and compliance reviews.
• Legal requirements — when required by law, such as reporting to public health authorities or responding to a court order or subpoena.
• Emergency situations — to contact emergency personnel or provide critical information necessary for your care in an emergency.
• Appointment reminders — to contact you regarding upcoming appointments or follow-up care.

All other uses and disclosures of your PHI — including marketing, research, or sharing with third parties not involved in your care — require your written authorization. You may revoke that authorization at any time in writing.

Under HIPAA, you have the following rights:

• Right to access — you may request a copy of your health records. We will respond within 30 days of your request.

• Right to an accounting of disclosures — you may request a list of instances where we have disclosed your PHI for purposes other than treatment, payment, or operations.
• Right to request restrictions — you may ask us to limit how we use or disclose your PHI. We are not required to agree to all restrictions, but we will comply when feasible.
• Right to confidential communications — you may request that we contact you in a specific way or at a specific location (e.g., a different phone number or address).
